Diabetes Management Program
We evaluate a person's condition and knowledge of the disease before recommending a plan of care, and make sure that appropriate counseling, peer support and special information on diabetes is readily available. We also provide classes on diabetes medications, coping with diabetes, nutrition (including eating out and label reading) and exercise. A nurse conducts a post-instruction evaluation, analyzes goals and makes recommendations for follow-up.
Pregestational and Gestational Diabetes Program
We also offer a pregestational and gestational diabetes program specifically designed to help those patients with pregnancy-induced glucose intolerance make safe, healthy and informed choices before, during and after pregnancy.
The program includes:
- Group classes offered on diabetes self-monitoring and nutrition.
- Patient follow-up as needed to include insulin instruction
Admission to the above programs requires physician authorization. The results of assessments and program outcomes will be provided to the referring physician.
