Levels of Care
The skilled rehabilitation unit at Baptist Rehabilitation-Germantown serves persons aged 14 and up and provides 24 hour nursing care with over sight from a rehabilitation physician who evaluates the patient and establishes the plan of care. Patients in skilled rehabilitation include those who have experienced a 3 day stay in an acute hospital within 30 days prior to admission. Patients with open wounds, dialysis, or IV antibiotics do not qualify for skilled rehabilitation. Patients in skilled rehabilitation typically receive 1.5-2 hours of therapy per day 5 days per week. Discharge goals include increased mobility and endurance for transition to the community or a more intense level of rehabilitation.
The inpatient rehabilitation unit at Baptist Rehabilitation-Germantown serves persons aged 13 and up and provides 24 hour registered nursing care. The rehabilitation program is led by a rehabilitation physician who is available 24 hours a day and will see patients at least 3 times per week but in many cases daily. Patients with various co-morbidities including wounds, dialysis, and IV antibiotics are eligible for admission. Patients in inpatient rehabilitation must require at least 2 therapies and be able to tolerate a relatively intense rehabilitation program generally defined as at least 3 hours therapy per day, 5 days per week or 15 hours of therapy over 7 days. Discharge goals include transition to home or other community setting with ability to safely function at the household level with or without caregiver support.
Next Step Program
The Next Step program at Baptist Rehabilitation-Germantown serves persons aged 13 and up through an intensive outpatient day treatment program. In this program patients generally participate in up to 6 hours of treatment 4-5 days per week. Treatment consists of traditional outpatient physical, occupational, and speech therapies as well as group treatment for community re-integration and carryover of skills learned in real life situations. Services are provided Monday through Friday from 8am-5pm. Discharge goals include safe and functional ability to actively engage and participate in life activities at the community level.
Traditional outpatient physical, occupational and speech therapy services are offered to patients of all ages in both pediatric and adult settings.
